Hospitals are notoriously mismanaged. The money pours in from lots of sources and goes out the window just a fast as it comes in and spent on bad decisions and funding a bloated, inefficient, and arrogant bureaucracy.
The last thing the VA needed was someone with years of hospital management experience.
Trump is a businessman. He applied what he learned in business to just about everything. Good things happening and results occurred but apparently people's feelings were more important to the left than getting the county on track including the VA.
