My suggestion was to swap the stock bed with a "flat bed" type bed. Not a farm truck bed, but a nice one with storage, etc. I personally like that look on a dually better than the factory bed. I would think the arms would clear that.
On another not, I think you need to be about 6'4" to reach any thing in a truck bed now even a half ton. I guess that is why GM and Ford have built in steps in the sides of their HD's now.

Don,
I have a 2002 39' GR with the "small" gooseneck, "A" frame tongue and inverted 5th. My truck is a 2012 2500HD 4x4. I dont have any clearance issues, EXCEPT when going over a berm.... Last winter, I was moving my car down to a fabricators place, and when I was backing in/turning over a berm/hump, the trailer connected with the box and tailgate.....ugh. It was a pretty significant hump in the side yard and pitch black night.... Otherwise, my 2012 has plenty of room and have never been close to hitting for the last 3 years. I also have airbags on it.
