They come off, just as described above. If you used head studs, they will have to come out too prior to head removal.

I was at the Indy Points Meet in 1978. I saw Fadely with Super Stock Maverick pull a head with the engine in the car and that was a Cleveland style head. I was close enough to see that he used a heavy rubber band to hold the 4 center head bolt up on the exhaust side.

I've done it several times at home and twice at Indy teardown barn, I use a old school 15" Craftsman swivel breaker bar.
