Good luck with your request.
I'm pretty sure the deadline was Nov. 19th. I learned 3 years ago the hard way....I sent in my request on the Thursday after the World Finals but Pat. C. sent me an e-mail saying the last day for submitting requests is the Tuesday following the World Finals
